To make a referral to British Home please send an email to Catherine Humphries, Registered Home Manager (catherine.humphries3@nhs.net), with the following details:

• Name, age and gender of the person being referred
• Relevant information about their health and the reason for referring them to British Home
• Details of which setting the person being referred is currently in
• Where known, who would be the principal funder of care at British Home
• Contact details of the person making the referral

All information received is handled in line with our privacy policy and details are held only for as long as a referral is in process.
